Most of us learned about the five senses: sight, hearing, smell, taste, and touch.
But did you know that humans have more than five senses? We actually have eight core sensory systems.
Interoception is another sense, which is our body’s inner sensory system. It helps us understand what is happening inside our bodies and recognize sensations such as hunger, thirst, temperature, pain, fatigue, a full bladder, and changes in our heart rate.
For people with ADHD, interoception can be a challenge.
You might only realize you're hungry once you’ve become irritable, tired, or completely drained from hunger.
Perhaps you don’t notice that you’re thirsty until you develop a headache or dry mouth.
Maybe you become so focused on what you’re doing that you don’t realize you need to use the bathroom until the sensation becomes urgent.
Emotions can also be difficult to understand. For example, anxiety can be difficult to distinguish from excitement.

As an individual with ADHD, you may experience these interoceptive challenges in a variety of ways. Some people may struggle with recognizing internal bodily signals, while others may experience more intense sensations.
ADHD can also affect attention regulation, time management, awareness, and working memory. These experiences can lead to interoception challenges and make it easy for a person to miss or postpone responding to signals coming from the body.
These challenges can make it difficult to regulate emotions, complete everyday activities, engage in self-care, or understand what our bodies need.

What Is Interoception?
Interoception is one of our sensory systems. While we often think of our senses as sight, hearing, taste, smell, and touch, interoception belongs to a larger group of senses that includes the vestibular system (your sense of balance) and proprioception (your sense of body position).
Your brain is constantly receiving information from your internal organs and bodily systems, and interoception helps you notice and interpret these signs so that you can respond to them.
It allows us to maintain homeostasis (internal balance) by monitoring any imbalances, recognizing them, and then motivating us to take action to get back to a balanced state.
For example, when your body becomes too warm, temperature-sensitive receptors detect changes in your internal temperature and send signals to your brain. Your brain interprets these signals as feeling hot, which can prompt you to seek out a cooler environment, remove a layer of clothing, or drink something cool.
Interoception also helps us recognize hunger, thirst, a full bladder, a need to have a bowel movement, fatigue, pain, heart rate, alertness, muscle tension, emotional states, and more.
Alongside noticing physical sensations, interoception also involves how we interpret and understand these sensations.
For example, you may not always consciously think, “My stomach is empty, so I need to eat.”
Instead, your brain may interpret these internal signals automatically and create the conscious experience of feeling hungry.
What Is Interoceptive Awareness?
Interoceptive awareness refers to your ability to notice and interpret signals coming from inside your body.
Everyone experiences this awareness differently. For some people, subtle changes in the body may be noticed very quickly. Whereas others may not recognize a sensation until it becomes more intense.
For example, you may notice that you are feeling thirsty and get a drink of water.
But someone else might become so focused on a task that they don’t notice they are thirsty until they experience a headache, dry mouth, or significant fatigue.
Another person might become deeply focused on a task and not notice their thirst until they are experiencing a headache, dry mouth, or significant fatigue.
This awareness of our bodily signals can also change depending on the situation. You may notice your body's signals easily when you are relaxed but have difficulty recognizing them when you are overwhelmed, highly focused on an activity, distracted, or experiencing sensory overload.
ADHD and Interoception
Interoception challenges can affect anyone, but differences in interoception and bodily awareness are more common in people with ADHD.
There is no single interoceptive experience that is universal to all people with ADHD. These experiences differ from person to person based on how our brains and bodies interact, but some common experiences include:
Difficulty noticing internal body signals
Experiencing certain sensations very strongly
Noticing certain signals more easily, while struggling to identify and interpret others.
ADHD can involve difficulties with working memory and task initiation. Even when you do notice a body signal, there can sometimes be a gap between noticing the need and actually responding to it.
You might recognize that you're hungry but think, “I'll eat after I finish this.”
This can create a cycle where body needs become increasingly urgent before they are addressed.
Hyperfocus and Missing Body Signals
A common trait of ADHD is hyperfocus, which can affect how attention is allocated. This is one of the reasons why interoceptive differences can be more noticeable with ADHD.
When you become deeply engaged in something, your attention can become focused on that activity, which can make it difficult to recognize the signals from your body.
You may start working at 10:00 a.m. and feel as if you have only been working for an hour. But when you look at the clock, it is actually 4:00 p.m.
During these hours, you may have missed bodily signals that were telling you that you were tired, hungry, thirsty, overwhelmed, or needed to change environments.
Hyperfocus is especially common when a task is highly engaging, interesting, or rewarding.
External supports can help you protect your basic needs while allowing you to engage deeply in the things that matter to you.
What Can Interoceptive Differences Feel Like?
Interoceptive differences differ depending on the person, situation, and sense. They can show up in many different ways.
You may have strong awareness of some body signals and limited awareness of others. Certain sensations may be experienced intensely, while others may barely be noticed.
For each of our senses, we may experience the following:
Hyposensitivity: Low sensitivity to internal signals, where sensations may feel muted or difficult to notice.
Hypersensitivity: High sensitivity to internal sensations, where signals may feel particularly strong, intense, or overwhelming.
Discrimination difficulties: Difficulty identifying or interpreting what a particular internal signal means.
You may experience one of these patterns, a combination of them, or notice that your experience changes depending on the situation.
For example, you might have difficulty noticing hunger but be highly sensitive to your heartbeat. If you recognize that something is happening in your body, you could have difficulty figuring out exactly what the sensation means.
Your interoceptive experiences may also change depending on your environment, stress level, sensory load, attention, sleep, and other factors.
Let's explore some examples below.
Hunger
Hyposensitivity to hunger signals may occur when you don't notice that you're hungry until the sensation becomes much stronger.
If you are engrossed and hyperfocused on your work or interests, your body's signals don't capture your attention.
Instead of noticing gradually that you are starting to become hungry, you may not realize that you need to eat until you are extremely hungry, shaky, irritable, tired, or unable to concentrate.
Even if you recognize that your mood and energy have changed, you may still not recognize why.
For some people with ADHD, hunger may also become easier to recognize once it reaches a more intense level.
Conversely, introception challenges can also show up as a hypersensitivity to hunger, where even mild hunger feels uncomfortable or distracting.
You may also experience discrimination difficulties, where you notice that something feels “off” but aren't immediately able to identify it as hunger. You may notice that you are anxious or overwhelmed but only realize later that you haven’t eaten, and this could be why.
Thirst
Thirst, like hunger signals, can be experienced in different ways.
If you are experiencing hyposensitivity, you may not notice that you need to drink water until you develop a headache, dry mouth, dizziness, or fatigue.
If you regularly forget to drink and miss thirst signals while focused on activities, external supports such as keeping water visible or within reach can help make it easier for you to respond to thirst before it becomes urgent.
With hypersensitivity, even a small change in thirst may feel particularly noticeable or uncomfortable.
You may also experience discrimination difficulties, where you notice physical discomfort but aren't sure whether you're thirsty, tired, overheated, or experiencing something else.
Bathroom Needs
Your ability to recognize and respond to bladder and bowel signals can also vary.
ADHD-related hyperfocus, distractibility, and difficulty shifting attention can make it harder to interrupt what you need when bodily signals are quieter.
This can make hyposensitivity or difficulty recognizing the need to use the bathroom until the sensations become more urgent more difficult.
You might tell yourself that you’ll finish something first but then realize that your need to urinate has become more urgent.
If you are in a state of hypersensitivity, you may notice bladder or bowel sensations quicker than others. Even relatively mild sensations may feel distracting.
You may also experience discrimination difficulties, where you recognize that something feels uncomfortable but have difficulty determining exactly what your body is signalling.
Fatigue
Challenges with interoception in relation to fatigue signals can also look different from person to person.
With hyposensitivity, you may not recognize that your energy is low until your energy has dropped significantly and you can no longer continue.
During these moments, you might keep working, studying, exercising, or engaging in daily activities without even noticing that fatigue is gradually accumulating. Your body is signalling that it needs rest, but your attention stays focused on what you’re doing.
Eventually, you may suddenly feel completely exhausted. This can be especially challenging if you tend to become highly engaged in activities that are interesting or stimulating.
On the other hand, hypersensitivity occurs when even small energy changes may feel very noticeable. It can make it difficult to determine whether you actually need rest or whether something else may be contributing to how you feel.
If you experience discrimination difficulties, you may know that something feels different but aren't sure whether you're tired, bored, overwhelmed, understimulated, or emotionally drained.
Pain
Pain signs can involve differences in sensitivity and interpretation.
If you are in a state of hyposensitivity, you may not notice that you have a minor injury or that you are experiencing physical discomfort, especially when your attention is focused elsewhere. This could explain why ADHD folks sometimes have bruises without knowing where they even came from.
When experiencing discrimination difficulties, you may notice that something hurts but have difficulty identifying where the sensation is coming from, what type of sensation it is, or what might be causing it.
Your awareness of pain may also change depending on what else is competing for your attention. You may be more aware of pain if your attention is focused on it.
Temperature
Temperature is another example of how interoceptive differences can vary.
With hyposensitivity, you might not notice that you are becoming too hot or too cold until the temperature changes significantly.
For example, you might continue wearing a sweater in a warm room without realizing how uncomfortable you have become. You could also fail to notice that you need another layer when you're cold.
If you are hypersensitive, smaller changes in temperature may become more noticeable. You may become easily distracted when slightly cold or warm.
With hypersensitivity, smaller changes in temperature may feel particularly noticeable or uncomfortable.
You may also experience discrimination difficulties, where you know that something feels uncomfortable but aren't immediately able to identify whether you're too hot, too cold, tired, overwhelmed, or experiencing another physical sensation.
Emotions
Emotions are often accompanied by physical experiences. For example, anxiety might involve a racing heart, tight chest, sweating, or stomach discomfort. Whereas excitement may involve increased energy.
As a result, interoception can also play a role in emotional awareness.
With hyposensitivity, you may not notice these physical changes until your emotional response has become quite strong.
With hypersensitivity, physical changes associated with emotions may feel particularly intense or overwhelming.
If you experience discrimination difficulties, you may notice that something is happening in your body without immediately understanding what it means.
You might think, “Something feels wrong, but I don't know what.” It could be anxiety, excitement, hunger, fatigue, frustration, sensory overwhelm, or another experience, but you aren’t able to figure out what the emotion you are experiencing is.
Learning to notice these patterns can help you begin connecting physical sensations with possible emotions and needs.
The Same Person Can Experience All Three
While it can be easy to think of interoceptive challenges such as hyposensitivity,
hypersensitivity, or discrimination difficulties as mutually exclusive, a person can experience all three together.
For example, you could have hyposensitivity to hunger, hypersensitivity to pain, and discrimination difficulties when it comes to identifying emotions.
Depending on the environment, attention, stress levels, sensory input, sleep, and other external factors, your experience of interoception can also change.
If you’d normally notice that you’re getting tired, you may completely miss these signals when you are deeply engaged in an activity that involves a special interest.
Understanding your own patterns can be useful for understanding the strategies you can use to support yourself.
What are the Impacts of Interoceptive Challenges?
Interoceptive challenges can differ from person to person, but they don’t typically only impact a single aspect of a person’s life. They usually have a ripple effect on multiple areas of your life.
When it is difficult to notice or interpret your body's signals, it may contribute to emotional dysregulation, sensory overwhelm, ADHD burnout, and difficulty recognizing when you need rest or support.
We’ll explore the impacts of interoceptive challenges in more detail below.
Emotional dysregulation occurs when a person has a difficult time managing intense feelings or how they react to them.
Your bodily signals and your emotions are deeply connected to one another. If you become overwhelmed or experience a spike in anxiety, your body may react to this. You might notice a racing heart, tense shoulders, and heavy breathing.
In these moments, your body is already responding to your emotions before you have had the chance to consciously identify them.
If you can recognize these signals of anxiety or stress within your body, it can help you regulate your nervous system before you become stressed or overwhelmed.
But if you experience interoceptive challenges, including hyposensitivity to emotions, you might not notice these emotional changes until they become intense. Because of this, you may be more likely to reach a point of significant distress before you have the opportunity to respond to it, which can make emotional regulation difficult.
ADHD itself can also involve differences in emotional regulation, including experiencing emotions more intensely and having difficulty shifting out of an intense emotional state.
If physical signals are difficult to recognize, it can become even more difficult to understand what you are experiencing internally.

Even though your body has been communicating all along that something is wrong and you need to rest, you may think, “I don't know why I'm suddenly so overwhelmed.
You simply may not have been able to recognize or interpret the signals yet. This is one reason increasing interoceptive awareness can be an important part of understanding your emotional experiences and improving emotional awareness.
If you have been working for several hours without eating or drinking, you may be feeling tired. There may also be a noisy environment, and you may experience frequent interruptions.
Even though you may not consciously recognize each of these factors individually, they may gradually accumulate in your nervous system.
Eventually, you may reach a point where you are more irritable, frustrated, emotionally flooded, and unable to continue.
For others, it can look like you are overreacting. But
internally, there may be unmet needs that have been building underneath the surface. These needs could include food, water, rest, movement, a break from demands, or time for transitions.
Developing ways to notice your body's signals earlier may help you identify some of these needs before they become overwhelming.
Interoceptive differences can make it difficult to recognize that you need rest.
As a result, you might continue to push through work, household responsibilities, or other everyday demands because you don’t notice how exhausted you are.
ADHD can make this especially difficult because you may rely on urgency or novelty to get things done.
Your ADHD brain may convince you that you can rest when you are finished with the tasks. But those tasks can take longer than expected. Before you know it, you have additional tasks that need to be completed.
Eventually, you may realize that you have been running on empty for much longer than you thought.
When you have been repeatedly missing or postponing your body’s signals, this can contribute to a cycle of pushing through, crashing into states of low energy, recovering, and then pushing through again. This cycle can push you into a state of ADHD burnout.
For people with ADHD, learning to recognize early signs of fatigue can help to develop a more sustainable approach to daily life.
How Can You Improve Interoceptive Awareness?
Interoceptive awareness strategies work by helping you develop gentle and practical ways to check in with yourself. Although these strategies will differ depending on the person, here are some strategies you can experiment with.
1. Create Regular Body Check-Ins
Instead of waiting until your bodily signals become strong, you can start by trying to create predictable moments throughout the day to check in with yourself.
During these moments, you can ask yourself:
Am I hungry?
Am I thirsty?
Do I need to use the bathroom?
Am I tired?
Am I too hot or too cold?
Am I experiencing pain or discomfort?
Do I need movement?
Do I need sensory input or less sensory input?
It may be easier to do a check-in if you use habit stacking, which would involve combining an existing routine, such as meals, meeting breaks, or getting ready for bed, with checking in with yourself.
2. Use External Reminders
If you regularly experience hyperfocus and become deeply absorbed in activities, external reminders can help you remember your body’s needs. External reminders can come in a variety of formats and may include the following:
Phone alarms
Visual sticky notes
Calendar notifications
Smartwatch reminders
Water bottles with time markers
These reminders act as accommodations that can help you compensate for differences in how your brain processes attention and internal signals.
3. Connect Body Signals With Needs
You may find it helpful to connect certain signals with specific needs. This is known as a personal “body dictionary.”
For example, if you have a headache, you can ask yourself if you need water, food, rest, or less sensory input.
When experiencing a racing heart, you may ask yourself if you are anxious, excited, physically active, or overwhelmed.
Irritability could be recognized as fatigue, hunger, overwhelm, or overstimulation.
Whereas difficulty concentrating can be seen as the need for food, water, movement, rest, or a quieter environment.
This body directory may help you build connections between physical sensations and possible needs so that it becomes easier to recognize your needs before your sensations become intense.
4. Make Basic Needs Easier to Access
Sometimes the ability to recognize a need is easier when you have easier access to something.
For example, to remember to eat, you can keep snacks readily accessible where you work. Some additional examples of easier access include:
Having a water bottle nearby
Putting sensory tools on your work desk
Keeping comfortable clothing available
Making easy-to-prepare meals
Creating a quiet space for breaks
Keeping movement options accessible
By reducing the number of steps between noticing a need and actually having to respond to it, it can make self-care more accessible.
5. Pay Attention to Patterns
Each of us has patterns between what physical sensation we notice and the need that is associated with it.
For example, perhaps you notice that you become irritable when you haven't eaten for several hours. Maybe you also notice that you become more sensitive to noise when you are tired.
Keeping a simple record of these patterns can help you identify connections between your body, environment, emotions, and needs. Over time, you may become more familiar with your own early warning signs.
6. Don't Rely Only on Internal Signals
One important part of supporting interoceptive differences is recognizing that you don't have to rely entirely on your body to tell you when something is needed.
External structure and routine can be just as valid. This means that you do not need to feel a need such as hunger, thirst, exhaustion, or a full bladder before responding.
Instead, you can respond to your needs proactively rather than waiting for your body to send a strong enough signal that you can't ignore it.
How Can Loved Ones Support Someone With ADHD and Interoceptive Differences?
If someone you care about experiences interoceptive differences, your support can make a significant difference.
Instead of telling the individual that they should just know when they are hungry or complaining that they aren’t noticing their needs, you can try approaching these situations with curiosity.
For example, you can ask them, “Would it help if I brought you something to eat?” or “Would you like me to remind you to take a break?”
External reminders can be particularly helpful when someone is deeply focused or overwhelmed.
However, support should still respect autonomy. Focus on providing supportive prompts and accommodations when they are wanted.
If you regularly struggle to recognize your body’s signals, you don’t have to figure this out on your own.
